Client Intake & Waiver

I voluntarily participate in massage therapy and/or Reiki sessions provided by Marie Benoit through ZenPath. I understand these sessions support relaxation and well-being and are not a substitute for medical diagnosis, treatment, or care. I have disclosed all known health conditions, injuries, medications, and allergies on this form, and I will inform my practitioner of any changes before each session.

I understand that massage therapy and energy work carry some risks, including temporary soreness, bruising, or discomfort, and I assume those risks. I agree to communicate immediately if any technique causes pain or discomfort. Sessions at Hickam Beach are held in a semi-outdoor setting and may include exposure to sun, sand, and uneven ground.

I release Marie Benoit and ZenPath from all liability for injury, loss, or damage arising from my participation, except where caused by proven gross negligence. I confirm I am at least 18 years old (or a parent/guardian has signed on my behalf), and I consent to receive the services I book.
